Home Health Care

Home health care is a wide range of health care services that can be given in your home for an illness or injury.
Home health care is usually given at no cost to the recipient as all services are covered by Medicare and most insurances.
Home care is more convenient, and depending on the particular issue, can be just as effective as the care you receive in a hospital or skilled nursing facility (SNF).

Skilled Nursing

A registered nurse who visits patients in the home.
The nurse works primarily in the area of secondary or tertiary care, providing hands-on care and educating the patient and family on care and prevention of future episodes.

Home Health Aides

A Home Health Aide is a trained and certified health-care professional who provides assistance to a patient in the home with personal care (as hygiene and exercise) and light household duties (as meal preparation) and who monitors the patient's condition while working closely with the registered nurse.

Physical Therapy

Physical Therapist (PT's) are highly-educated, licensed health care professionals who can help patients reduce pain and improve or restore mobility - in many cases without expensive surgery and often reducing the need for long-term use of prescription medications and their side effects.

Speech Therapy

Speech Language Pathologist (ST, SLP) is a highly-trained professional who evaluates and treats patients who have difficulty with speech or language.
An SLP or ST is a licensed healthcare provider who evaluates and treats patients with speech, language, cognitive-communication and swallowing disorders in individuals of all ages, from infants to the elderly.

Occupational Therapy

Occupational Therapist (OT's) are highly educated, licensed healthcare professionals that through use of assessment and treatment to develop, recover, or maintain the daily living and work skills of people with a physical, mental, or cognitive disorder.

Medical Social Services

Medical Social Workers play a vital role in the care team. At times, your physician and/or patient or family caregiver may request to a Medical Social Worker to conduct an assessment to help determine patient needs and identify any limitations, barriers, or safety issues that might impede recovery. An MSW also educates patients and families regarding community resources and support, and assists with long range planning, including advance care planning and directives, and overall care opportunities. Our Medical Social Workers work with the patients and families to keep patients at home in a safe and healthy environment. Medical Social Services include:

  • Community resource planning and coordination
  • Advance directives (Power of Attorney for Health Care or Living Will)
  • Assistance in emotional adjustments to life changes
  • Crisis intervention
  • Assistance with applications for financial aid
  • Counseling with life-style changes
  • Teaching regarding coping with loss and change
  • Grief counseling/coping with impending loss

Personalized Patient Education

Patient Education is a significant component of home healthcare. It is a planned, systematic, sequential, and logical process of teaching and learning provided to patients and patients families in the home setting.
Patient teaching and education are essential components of the nursing process.
Data indicates that when patients are provided with a solid knowledge base about their disease process and treatment, the outcome is more favorable.

Specialization in Diabetic Care

Living with diabetes can be less of a challenge with proper support, care and education.We work closely with your physician to offer your loved one the diabetic care plan that meets their needs.
Due to the high incidence of diabetes in our community, we have taken extra steps to give you and your loved one specialized diabetic care. Our services are performed conveniently in the home, utilizing all the latest home care methods and technologies.
Components of the program include:

  • Skilled care as ordered by physician
  • Assessment of patient’s health status
  • Nutrition education specific to Diabetes
  • Patient teaching based on proven educational techniques, tailored to individual needs and emphasizing the development of self-care management skills.
  • Coordination and communication with the patient’s physician and all health care providers involved in the patient’s care.
  • Outcomes reporting on key indicators of diabetes self-management and patient satisfaction.
  • Work with suppliers to provide patients with glucometers specific to their needs such as talking and no coding glucometers.
